class=\"H1_Templat\">\u2022 UIL Acknowledgement of Rules<br \/><\/span><span class=\"H1_Templat\">\u2022 UIL Anabolic Steroid Testing Agreement<br \/><\/span><span class=\"H1_Templat\">\u2022 UIL Concussion Acknowledgement Form<br \/><\/span><span class=\"H1_Templat\">\u2022 UIL Sudden Cardiac Arrest Awareness Form<br \/><\/span><span class=\"H1_Templat\">\u2022 RISD Extracurricular Activity Acknowledgement &amp; Agreement Form<\/span><\/p><p><span class=\"H1_Templat\">All forms should be submitted electronically\u00a0<em>except for the <a
